CAP is the community alternatives program for disabled adults. It is a Medicaid funded program which provides home care as an alternative for those facing nursing facility placement.

2. How do I qualify for CAP?

Qualifications include:

• You Have to Be 18 Years of Age  or Older with Disabilities
• Live in Private Residential Settings
• Need Help with Personal Care
• Eligible for Cap Medicaid
• At Risk for Nursing Home Placement
• Can Have Their Health, Safety, and Well-Being Needs Met a Home Withing Allowable Medicaid Cost Limits

4. How many hours of In-Home Aide Services can I receive?

The number of hours an individual can receive is depended upon an in-home assessment that will be completed by a social worker and registered nurse.
